まず2025年問題ってなんのことでしょうか？
団塊の世代の人々が75歳以上である後期高齢者になり、日本が国民の4人に1人が75歳以上である調高齢者社会を迎える2025年。
日本では戦後すぐの1947年-1949年の年代に第一ベビーブームと言われる出生率の急上昇が起こりました。その年代に生まれた人を団塊の世代と呼んでいます。
現在高齢者が増加する一方で、女性が一生のうちに子供をどれだけ生むかを示す割合の出生率は減少しています。つまり高齢者は増えるのに、それを支える若者は減っていると言うことです。2021年現在の後期高齢者は1871万人で、2025年には2200万人になると予想されています。高齢者が増え、社会や企業に影響を及ぼすため”2025年問題”と言われ、世の中ののあり方が変わろうとしています。
そのため、2021年現在では医療や人口の変化について2035年、2040年とさらに先を見据えての対策を検討しています。(*1)
なぜ問題なのか詳しく教えてください、介護にはどう結びつくの？医療も関係あるの？
高齢者が増えると言うことは、介護サービスを利用する高齢者も増えることになります。しかし生産人口と呼ばれる、働く若者が減っていくわけですから、サービスを提供できる人が少なくなる＝人材不足となることが予想されるわけです。
介護用ロボットなどのITの導入も期待されますが、経費の面で多くの人に導入するのが困難です。
また5年以内に実用化がどこまでできるかも不透明だと言わざるを得ません。
不足を心配するのは人材だけではありません。介護サービスは介護保険を使い利用されることが大半であるため、その介護保険を使う人が増えると言うことは財政の圧迫にも繋がっていきます。
現在でも特養のような人気の高い公立の施設は、入所を希望しても入所ができず、先の見えない待機を余儀なくされている人がいます。
他の施設に入所すればよいと思うかもしれませんが、経済的な面からそう自由に選択できない人が多いのが現状です。そしてその数はますます増えていくと考えられます。
介護と関わりのある医療も多大な影響を受けます。例えば病院のベットが埋まっており、必要な時に入院することができないことや医療施設の不足、医師をはじめとした医療従事者の不足と言った問題が予想されています。
他にはどんなことが2025年問題に関連してくるの？
前述の項目でも説明しましたが、介護に関連する内容で下記のような点も把握しておくべき点であると言えます。
・年金の給付額引き下げ/受給年齢引き上げの問題
・社会保障費を補填するための増税の可能性の懸念
・病院にかかった場合の窓口負担の増加
・介護以外の仕事も人材の不足の恐れ/中小企業存続の危機
これを見ると、「家族には要介護者がいないから、2025年問題は関係ない」と言いきれないことがわかりますね。それどころか、誰にでもかかわってくる問題が大半で、他人事ではないことが明らかなテーマです。
2025年問題に備え、国はどんな対策をしているのか
ここまでの話から先を考え、暗くなってしまったかもしれません。しかし、これらの問題は放置されているわけではありません。2025年まで“残り4年”と迫っている現在、国は2040年と先さらなる未来を見通し、下記の4点を主に重視し、前向きに取り組みを行う内容を発表しています。ここではその一部を抜粋しまとめます。(*1)
● 多様な就労の手段、社会参加を増進
⇒70歳までの多様な就労の機会を増やし、成果に応じて評価や賃金を提供できるよう環境づくりを行います。また就職氷河期時代のキャリア形成や就労支援を強化する若年者についての強化ばかりでなく40代以上の人に向けた整備も検討していきます。
● 健康寿命をのばすこと
⇒生活習慣病になることや、その重症化を防ぐための取り組みをおこなっていきます。
具体的には医療機関とスポーツクラブと言った民間企業が連携し、運動や栄養についてのプログラムを提供します。それにより個人間の生活習慣の変容に働きかけます。
● 福祉医療の改革
⇒福祉医療の場における人材の減少に対応するため、AI・ロボットの導入やIT技術を活用を進めていきます。また就労の件にも共通する点となる、シニア層を医療介護の人材として活用すること、医療の現場での業務内容を業務ごとに仕分けをするように計画をしています。
● 給付と負担の見直し等による社会保障の持続可能性の確保
⇒前世代の社会保障制度が確実に行えるように、これまでの社会保障改革を見直します。高齢者医療や介護制度を本人の能力や資産に応じバランスを取って負担を検討するなど10項目にわけ取り組んでいきます。
介護関連のことについて、個人でどんなことをすればいいの？
個人単位では、生活習慣の改善、疾病の早期予防、介護予防がなにより重要な点となります。今は関係ないと思われる健康な体も、半年、一年と年を追うごとに加齢していくことは避けられません。
厚生労働省の作成した『介護予防のための基本チェックリスト』 (*2)をチェックしてみましょう。
このチェックリストの項目を確認することによって、の自分自身で取り組む必要性のある予防行動がわかります。例え同じ年代でも対策するべき内容は人それぞれ、これを機にチェックリストを活用してみてはいかがでしょうか。
