毎年2月半ば〜3月半ばまでは確定申告の時期です。
年金生活の高齢の方は基本、年金収入が400万以下であれば、確定申告はしなくてよいことになっています。
しかし所得税・住民税を支払っているなら、確定申告をしたほうがよいとお勧めしたい方もおられます。
- 病気があって、医療費の負担が大きい方（200万円以上の年金額なら年間10万円以上）
- 障害者手帳をお持ちの方
- 要支援、要介護のなんらかの介護度が出ておられる方などです。

確定申告をすると得することがある！？
要支援２の介護認定がおりている85歳のＡさんは、２歳年下の要介護１の妻のＢさんと二人暮らし。
お互いに足が不自由で歩きづらいだけで認知症はありません。妻のＢさんは椎間板ヘルニアで腰痛があり家事が負担になっています。ふたりは週に一度、ヘルパーさんにお掃除とお買い物にきてもらい、Ａさんは体力を落としたくないと運動のために週に２回午後から半日だけのデイサービスに通っています。
Ｂさんは家でお風呂に入ることが難しいため、週に3回、１日デイサービスに通い、週２回は、訪問リハビリに来てもらっています。
いつかまた、夫のＡさんと趣味だったウォーキングができるようになることが、妻のＢさんの目標です。
年金も夫婦で食べていくのがやっとぐらいの金額だし、面倒くさいので確定申告なんかしていません。
しかし昨年は大変なことが起こりました。妻のＢさんがお風呂場で転んで右手を骨折し、二か月間入院したのでした。それいらいＢさんはデイサービスでお風呂に入るようになります。
医療費やら、デイサービスの料金や食事代などで何かと出費がおおくなり、少しでも取り戻せないかと思った夫のＡさんは、思い切って今年は確定申告をしてみることにしたのです。
『高齢者』なのになぜか『障害者』控除 申請書を出すだけで最高40万円の控除が受けられる
そこでＡさんは、以前からケアマネージャーさんが勧めてくれていた「障害者控除申請書」の用紙を、介護保険証のコピーをつけて、市役所の介護保険課に出してみることにしました。
昨年もその前の年も「わしら、障害者とちゃいますし、めんどうなのでやりませんわ～」と断っていたのです。でも大丈夫。過去5年分まではさかのぼって申請することができるのです。
案外知られていませんが、税法上では要介護者＝障害者という控除に分類されます。
そして、市町村によって違いはありますが、著者の住む市では、要支援でも要介護でもなんらかの認定を受けていれば『障害者控除』の対象になるのです。※詳しくはお住いの市町村の介護保険課にお問い合わせください

と、ＡさんＢさんご夫婦の場合、会社勤めだった夫の厚生年金が月に1９万円。専業主婦だった妻の国民年金が月に5万円です。
夫婦の月収は月に24万円で、夫だけの年収は228万円。
年金に所得税や住民税などの税金が課税されるボーダーラインは158万円といわれます。
妻のＢさんは年収60万円なので税金はかかりませんが、夫のＡさんはボーダーラインを超えるので税金を支払っていることになります。
そこで、要支援２の夫と要介護１の妻の障害者控除を適応すると 27万円×２で－54万円で控除後の所得は174万円まで下がります。
えっ？と思う意外な費用が医療費控除に入ります。
昨年は妻が骨折して入院したため、その後の通院やリハビリで医療費が高額になってしまったＡさんＢさんご夫婦。夫婦二人で月１万円程度だった医療費が一年で20万円かかってしまいました。
先ほどの174万円からさらに医療費を控除して154万円。これでギリギリ課税されない金額まで控除後の年収が下がりました。
さらに妻のＢさんは、訪問リハビリという医療系のサービスを使っているので月6千円の費用と、さらに週に3回通っているデイサービスの費用月9千円も医療費控除の対象となるのです。

居宅療養管理指導という医師や薬剤師が訪問して指導している場合

これらのサービスを利用している場合、本来なら医療費控除に含まれないディサービスの費用や（食事代をのぞく）ヘルパーさんの身体介護の費用も控除されるのです。
Ｂさんの訪問リハビリ月6千円、デイサービスの費用月9千円。ヘルパーさんの費用は残念ながら生活の支援なので含まれませんが、年19万2千円がさらに控除されて、所得は切り上げても135万円に。
税務署でないと、詳しい税金の計算方法はわかりませんが、ざっと計算しただけでも確実に非課税になれるのです。
月々に支払う介護費用もかえってきました！
さて、夫のＡさんが非課税になったおかげで、もともと国民年金だけで課税されていない昭子さんと2人。見事に非課税世帯になりました！
さらに非課税世帯になると、高額介護サービス費の上限がさがります。
高額介護サービス費とは１～５段まであり
第1～2段階 上限15000円（生活保護または年金80万円以下の個人）
第3段階 上限24600円（市民税非課税世帯）
第4段階 上限37200円 (課税世帯520万円以下）
第5段階 上限44400円 (それ以外の世帯）
妻のＢさんのデイサービスと訪問リハビリ、ヘルパーさんの支払い金額の合計は、月に16200円（1割負担）
夫のＡさんのヘルパーさんと運動型デイサービスの支払い金額の合計は、月に5000円（1割負担）
合計21200円なので、残念ながら第３段階の世帯で24600円には届きません。しかし昭子さん個人の支払金額は第1～2段階15000円をこえているため、今年度からはオーバー分の1200円があとから払い戻されることになるのです。
その他にも
非課税世帯になると、施設に入所したりショートスティを利用する際の、部屋代や食費なども安くなります。
さらに医療費の自己負担限度額や、支払った医療費と介護サービス費の合計の金額の負担上限額も低くなります。
つまり、あらゆるサービスに支払うお金の天井が低くなるのです。
医師の意見書があればオムツ代も医療費控除に含まれるので確定申告できます。
また、非課税世帯であれば一定の条件を満たせば5000円相当のオムツが送ってもらえることも！？
ご高齢のご両親は情報をキャッチする機会が少なく、複雑な制度を理解することはなかなかの困難です。
