3.該当の市区町村で定める所得を下回る方
4.該当の市区町村の高齢者補聴器支給事業の現物支を受けていない方、または、購入費助成を受けていない方
■申請から支給までの流れ
担当課の課へ、事前に連絡をします。
1.該当の市区町村で要件を確認後、申請書も提出する必要があります。
2.申請受付後、要件を満たしている方は「補聴器検診依頼書」と「補聴器の支給案内（指定耳鼻咽喉科一覧）」をもらいます。
3.該当の市区町村指定の耳鼻咽喉科にて検診を受けて受けます。
4.検診の結果により、補聴器が必要な状態であると医師が判断した場合、耳鼻咽喉科にて、補聴器の支給が行われます。
5.申請に必要な書類⇒補聴器支給申請書
転入された方で、該当の市区町村にて所得が確認できない場合は、上記書類以外に証明する年度の「1月1日現在」の住所地の所得証明（住民税非課税証明書等）が必要となります。
一度、補聴器支給申請書を提出すると、購入費助成への変更はできないので注意が必要です。
■補聴器の購入費用、部分助成の場合
1人1回1台限り、30,000円を上限に補聴器本体の購入費を助成してもらうことができます。
購入金額が30,000円未満の場合は実費分の助成になることを覚えておきましょう。
対象者
次のすべてに該当する方
1.該当の市区町村にお住まいの65歳以上の在宅の方
2.補聴器の支給を受けていない方(障害者総合支援法による)
3.定める所得を下回る方（下記図表を参照）
4.該当の市区町村の高齢者補聴器支給事業の現物支を受けていない方、または、購入費助成を受けていない方
