「今のケアマネジャーとどうしても気が合わない。」 ケアマネジャーの変更はできますか?
両親の生活が心配になり、介護保険サービスの利用を検討する際、いろいろと相談したり、手続きの申請を手伝ってくれる、心強いケアマネジャー。
インターネットや様々な情報を得ながら、ケアマネジャーの事業所を慎重に選んだつもりだけど、自分と合わない部分が目に付いたり、両親に対してしっかり対応をしてくれない事が多かったりと感じた事はありませんか？
もちろん、ケアマネジャーも人間ですから、色んな人がいます。
今回は、そのような時、「ケアマネジャーを変更できるのか？」という疑問から話を進めていきたいと思います。
目次
ケアマネジャー選び
ケアマネジャーを変更できるのか？という疑問ですが、結論から言いますと「可能」です。
実はケアマネジャー変更についての規定等は、契約の際に交わした契約書や重要事項説明書に記載されています。
ケアプランセンターやケアマネジャーを選ぶ際、どんなに評判が良いケアマネジャーだったとしても、必ずしも相性がいいわけではありません。
男性と女性というだけでも、話しやすさは変わってくるものです。
また、相性の合わないケアマネジャーを無理に利用し続けるとストレスが溜まり、相談が出来なくなってしまうケースなどもあります。
そうなると、あまり良い状況とは言えません。
ケアマネジャーの変更を考える時
では具体的にどのような時にケアマネジャーを変更したいと思うのか、例を挙げたいと思います。
参考例
・家族の意見をあまり聞いてくれず、自分の意見ばかりで話を進めていく。
・「また連絡します」と言ったきり、折り返し連絡がない。
・「どうしましょう？」と聞いてばかりで、提案してもらえない。
上記のように、意見等を聞き入れてもらえない事や、真摯に向き合っていないことが、ケアマネジャーの変更を考えるきっかけになっているようです。
ケアマネジャーは、一人に対して数十人の利用者を担当しています。
書類の作成や、訪問調査、介護保険の給付の管理など、業務は極めて多忙です。
だからといって両親の意見や家族への対応を後回しにしていると、良いケアマネジャーとは言えません。
担当してもらっている以上、しっかりと話を聞いて対応してほしいというのは当然の事でしょう。
ですが、ケアマネジャーも一人の人間であり、何らかの事情が重なっている場合もあります。
コミニケーション不足により、お互いの気持ちの掛け違いの可能性もあります。
もちろん話し合うことで、お互いの気持ちを理解しあい、関係を修復できれば、それに越したことはありません。
すぐに変更を考える前に、まずはケアマネジャーと話し合いをする機会を設け、こちらの意向を伝えましょう。
それがかなわない場合は、ケアマネジャーが所属している事業所に連絡を入れ、管理者に話をしてみるのもいいかもしれません。
ケアマネジャーの変更手順
一番スムーズにケアマネジャーを変更する場合は、紹介してくれた所へ再度相談してみる事です。
参考例
・地域包括支援センターに相談を行い、紹介をしてもらった場合、再度相談をする。
・役所などでリストをもらい、自身でケアマネジャーを選んだ場合、地域包括支援センターに相談し、新たなケアマネジャーを探す。
上記のように、直接、事業所や担当ケアマネジャーに伝えなくても、地域包括支援センターに相談を行うと、スムーズに変更ができます。
包括支援センターの職員は、地域のケアマネジャーと交流があり、多くのケアマネジャーを知っている為、しっかり相談に乗ってくれ、アドバイスや話し合いを行う事ができます。
ケアマネジャーの変更を相談した後は、新しいケアマネジャーを紹介してもらい、アポイントメントを取り顔合わせをするといった流れになります。
特に問題がなければ、契約を交わし新しいケアマネジャーの利用開始となります。
ここまでがケアマネジャーの変更の手続きとなります。
変更時、気を付けるポイント
１.現在使用しているサービスを継続できるように伝える
２.事業所を変更するときに、変更理由を的確に伝える。
３.家族が行う場合は、必ず両親の承諾を得ておく。
４.変更のデメリットをしっかり把握しておく。
特に気を付けるポイントは1番と4番です。
1番ですが、ケアマネジャーを変更する際、現在のサービスが使えなくなることがあります。
変更する際には、そうならないように変更前のケアマネジャーから新たなケアマネジャーにしっかり情報の引継ぎをしてもらう必要があります。
また、これを機に、変更したいサービスや追加したいサービスなどがある場合も、上記同様、新旧ケアマネジャーに相談し、サービスを１から組みなおしてもらうのも一つの手です。
4番ですが、ケアマネジャーを変更する際、以下のデメリットが発生します。
デメリット
・新しい事業所との契約が必要であり、手間がかかる。
・状況把握の為、利用者の現在の状況などを一から伝えなければいけない。
・サービス担当者会議を開かなければならない。
・１から信頼関係を築き、コミニケーションをしっかりとる必要がある。
デメリットをしっかり把握し、理解した上で、変更後の事を考えておくこと必要があります。
まとめ
ケアマネジャーとは、困ったときに相談できる関係性でなければいけません。
現在のケアマネジャーとコミニケーションを取れていないから相談しづらい、相談してもどうせ解決しない。
等の理由で諦めてしまうような関係性であれば、ケアマネジャーを変更した方がいいでしょう。
ですが、現在のケアマネジャーとしっかり話し合いを行い、コミニケーションを図ることで解決できるのであれば、まずはそうする事をお勧めします。
ケアマネジャーを変更したからといって必ずしも良い結果になるとは限りません。
まずは自身で出来る事から行い、それでも無理だと思う時に、初めてケアマネジャーの変更を考えてみるのも良いかもしれません。
